How to register

If you wish to register at Chapel Street Surgery please come to the practice and collect a registration pack from reception. You will be asked to fill it out as much as possible but an NHS number is needed. If you do not know your NHS number then you can call your current practice who will be able to provide this for you. If you have repeat medication then you can find it on a repeat slip.

We will ask for two forms of identification one needs to be photographic evidence like a passport, driving license or bus pass. The other is a proof of address, for example a council tax bill, phone bill etc.

Please do not put your forms through our letter box. The reception team need to check that everything is filled out with you present.

Please also be aware that if you join out practice and you are taking benzodiazepines, sleeping tablets or high strength pain killers then you will need a review with a GP and maybe started on a reduction regime.
